1. There were two swords. The Sword of Fate and the Sword of Doom. (or vice versa, with Doom first and Fate second, I'm not sure about that.
2. Earth, Air, Fire and Water.
3. Miyumi. I dunno if her second name is said, but if it is, I can't remember it.
4. Miss Mirimoto, Sojo, who worked for her, kinda these robot ninjas and, in the end, the "Super villain" was the Black Samurai.
5. The villains lock Fred, Daphne and Velma up in a (I don't really know what it is, or the word you want here...) storeroom (?), so they can't get away.

The dragon was green, as depicted in almost all the symbols of him as well.
